In the dissertation, you and your church will have complete anonymity.  It will be impossible to connect any of this information with your church field.  Therefore, this opening page is optional.  If you feel better not providing names, I understand, and this report will not be hampered by the absence of that information.  However, if you will provide me with this information, it will assist me in case I feel a need to contact you for clarification.
